Who Are We?

Primary Healthcare Darlington was established in 2014 as the local GP federation consisting of 11 GP Practices, 57 GP’s covering 107,000 registered patients in Darlington, County Durham. We have close links with Darlington Borough Council and local NHS Foundation Trusts and have the full support and backing of Darlington CCG.

The formation of our company has enabled the 11 Practices to work together at scale to ensure consistent and equitable services for the registered patients of Darlington. We have the great advantage of using the same clinical system, SystmOne, which enables all Practices to work as one when using clinical templates and pathways.

I worked in Darlington at Neasham Road Surgery as Business Manager for over six years. I have over 20 years’ experience as an IT Consultant and am also Director of a Management Consultancy Company. Within PHD I have responsibility for Finance and Communications. I am married to a Management Consultant with two teenage children.
Sally Hutchinson

I have worked in the NHS for almost 30 years; the last 10 years as a Practice Manager in Darlington. I am passionate about quality services for patients and ensuring these are provided in Darlington, for the patients of Darlington. My role within the company is Contracts and Contract Monitoring. In my spare time, I enjoy walking my border collie, Meg and partaking in the social life in and around Darlington!
Dr Chris Mathieson

I have been a GP in Darlington for seven years. Prior to this I worked in Accident and Emergency Medicine in Teesside. I still maintain an interest in Urgent Care and have done much work in this area for Darlington CCG, before becoming a Director for PHD. I have lived in the North East for more than 20 years after an education primarily in Scotland. I am passionate about developing the best primary medical care for the patients of Darlington. Outside of work I enjoy golf and football.
Dan Palmer

I graduated from Manchester University in 2005 after completing the St Andrew’s-Manchester medical degree. I worked in various posts in North Tees and Hartlepool before moving to New Zealand in 2007 where I worked as a GP. I returned to the UK in 2009 and completed the UK GP training programme in 2012. I am also a GP trainer. I have a particular interest in sports medicine and currently work with local football clubs. In my spare time I enjoy keeping fit and have a busy family life with my two young children.

I have been a GP Partner in Darlington for three years. I became a Director of PHD as I am committed to maintaining high quality care in Darlington. I am firmly committed to the area having grown up locally and now reside here with my family. Despite coming away with new injuries each week I still enjoy playing football in a local league, but above all I enjoy spending my spare time with my two young children.

Services

Following a successful bid for the Prime Ministers Challenge Fund, Primary Healthcare Darlington are able to provide the following services for the patients of Darlington.

Prebookable Weekend Surgeries

Available to all registered patients of Darlington who struggle to attend their surgery during the working week due to work commitments, caring responsibilities or need to be accompanied by a carer. These appointments are available on a Saturday between 8.00am and 2.00pm and are booked through the patient’s own surgery. The Saturday clinics are held at Denmark Street Surgery Annexe with parking and pharmacy facilities on site. The clinics are covered by local Darlington GPs, Practice Nurses and Health Care Assistants who, with your consent, will have full access to your medical records to help with your ongoing care.

Evening Telephone Advice

Monday to Friday during the hours of 6.00pm and 10.00pm, local GP’s provide a telephone advice service as part of the NHS 111 service to all patients registered with a GP practice in Darlington. GP’s can provide self-management advice, issue a prescription to be collected from the patient’s registered surgery the following day, arrange next-day appointments for patients at their own practice or arrange a face to face appointment at the Urgent Care Centre that night.

Hospital to Home Service

PHD are piloting a Sunday service run from the Acute Medical Admissions Unit at Darlington Memorial Hospital. A small team of local GPs are supporting consultant colleagues on the ward in facilitating early but safe discharges, ensuring the necessary health and social care provision is available to meet individual patient needs.
